If you’re looking for a local, welcoming health service in the Far West of NSW, Coomealla Health Aboriginal Corporation (CHAC) is here for you. We are an Aboriginal Community Controlled Health Organisation (ACCHO) providing culturally safe, community-first primary healthcare and wellbeing programs from our clinics in Dareton and Wentworth.


CHAC supports community members across the region — including Buronga, Curlwaa, Yelta and Mildura — and we welcome all members of the public.

Community-Controlled, Culturally Safe Care


CHAC is led by community and focused on delivering healthcare that is respectful, safe and practical. Our approach is holistic — meaning we support your physical health, social and emotional wellbeing, prevention and long-term care.


Whether you need a GP appointment, nursing support, chronic disease care, or help connecting to other services, our team is here to help.


Our Clinics: Dareton & Wentworth


We operate two clinic locations:

  • CHAC Dareton – serving Dareton, Buronga, Curlwaa and surrounding communities

  • CHAC Wentworth – serving Wentworth, Yelta and the wider region, including connections to Mildura services

GP Health Services


CHAC provides general practice care delivered by our General Practitioners in collaboration with our Aboriginal Health team and nursing staff. GP services include:


  • Health checks and general consultations

  • Treatment for acute illness and injury

  • Chronic disease management (e.g., diabetes, heart health, respiratory health)

  • Care plans and coordinated follow-up care

  • Women’s health and men’s health

  • Family planning and referrals

  • Referrals to allied health and specialist services

  • Ongoing support to help you stay well


If you have multiple concerns or complex needs, we can help you book a longer appointment.


Aboriginal Health & Nursing Support


CHAC’s model of care includes strong support from our Aboriginal Health Practitioner and Aboriginal Health Workers, working alongside nurses and midwifery services. This team can assist with:


  • Health checks and care coordination

  • Support with follow-ups and reminders

  • Chronic disease monitoring

  • Culturally safe support to navigate appointments and services

  • General nursing care and health education


This team-based approach helps patients feel supported and connected — especially when managing ongoing health needs.


Allied Health Services (including Visiting Services)


Allied health services support whole-of-person care and may include help with nutrition, diabetes education, mobility, hearing and vision.


At Dareton, allied health services may include:


  • Dietitian

  • Diabetes Educator

  • Podiatry (visiting)

  • Audiology (visiting)

  • Optometry (visiting)

  • Speech Pathology (when available)


Allied health availability can vary by clinic and visiting schedule — contact reception to confirm appointment times.


We also welcome visiting allied health and specialist providers, and CHAC may be able to support room use by arrangement.


Community Programs & Wellbeing

CHAC runs community programs that strengthen connection, wellbeing and healthy living. Programs may include:


  • Women’s Health Programs

  • Men’s Health Program

  • Elders activities and lunch program

  • Mum’s & Bub’s

  • Kumpa Kiira (community awareness and wellbeing support)


Programs vary throughout the year depending on community needs. Contact reception to find out what is currently running and how to join.


Bulk Billing and Patient Rights


CHAC services and programs are bulk billed and we aim to keep care accessible for our community. Please bring your current Medicare details to appointments.

CHAC supports the principles of the Australian Charter of Healthcare Rights, including your right to:


  • Safe, high-quality care

  • Respect, dignity and culturally safe communication

  • Clear information about your care and choices

  • Participation in decisions

  • Privacy and confidentiality

  • Providing feedback and making a complaint
