Certificate of Aboriginality
CHAC can issue Certificates of Aboriginality to eligible community members, in line with our responsibilities as an Indigenous corporation under the CATSI Act 2006.
Service Offerings
Coomealla Health Aboriginal Corporation (CHAC) is an Indigenous corporation listed under the CATSI Act 2006. The CHAC Board of Directors may issue Certificates of Aboriginality to known community members to help confirm Aboriginality for relevant purposes.

When considering an application, the CHAC Board of Directors will assess the information provided and may establish the applicant’s credentials through any means available. If Aboriginality cannot be confirmed, the application may be declined and a certificate will not be issued.

Fill out the application with as much detail as possible.

Step 3 — Lodge your application
Return the completed form to CHAC reception. Your application will be tabled at the next Board of Directors meeting.

If you cannot print the form, please visit CHAC and our staff will assist you to complete it.
